Liability Waiver Agreement

By participating in Piece Of Your Time classes, I hereby agree that:

  1. I understand that yoga includes physical movements as well as an opportunity for relaxation, stress reduction and relief of muscular tension. As is the case with any physical activity, the risk of injury, even serious or disabling, is always present and cannot be entirely eliminated. If I experience any pain or discomfort, I will listen to my body, discontinue the activity, and ask for support from the instructor. I will continue to breathe smoothly. I assume full responsibility for any and all damages, which may incur through participation. 
  2. I understand that yoga is not a substitute for medical attention, examination, diagnosis or treatment. 
  3. If I am pregnant, become pregnant or I am post-natal or post-surgical, I affirm that I have my physician's approval to participate.
  4. I also affirm that I alone am responsible to decide whether to practice yoga and participation is at my own risk. 

I hereby agree to irrevocably release and waive any claims that I have now or may have hereafter against Nikola Jurcakova.

By participating in Piece Of Your Time classes I accept this Liability Waiver Agreement voluntarily.

1)  What are cookies

1) Personal data administrator

a) The administrator of personal data, Piece Of Your Time, Nikola Jurcakova, (hereinafter referred to as the "administrator"), declares that all personal data processed by the administrator are strictly confidential. The administrator shall handle them in accordance with national and European Union law in the field of personal data protection.

b) The administrator collects, stores and uses your personal data in the sense of Act No. 110/2019 Coll. on the processing of personal data (hereinafter referred to as the "Personal Data Processing Act"), Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EA ("GDPR Regulation"). The individual purposes for which the administrator processes personal data are further defined.

c) The Administrator also collects this personal data through their site at http://pieceofyourtime.com/enae-yoga/ (the "Site").

d) This policy is issued by the administrator so that you are sufficiently informed about what personal data the administrator processes, for what purpose, for how long, who will have access to your personal data and what your rights are. This policy applies to all personal data collected by the administrator, whether collected for the purpose of fulfilling a contractual relationship, a legal obligation, a legitimate interest, or given consent.

2) Processed data

a) The administrator is entitled to process the personal data you entrust to them for the fulfillment of these purposes.

i) Provision of services, performance of the contract

(1) Personal data: Your name and surname, residential address, email address, telephone contact.

ii) Bookkeeping

(1) Personal data (billing data): Your name and surname, residential address, email address, telephone contact, bank account number.

iii) Statistical purposes

iv) Marketing, sending newsletters

(1) Personal data: Your name and surname, email address.

(2) If you are not a paying customer, we send you newsletters only on the basis of the consent granted when providing your personal data to the newsletter form.

(3) In both cases, you can withdraw your consent to receive the newsletter by using the unsubscribe link in each email sent to you.

v) Legitimate interest

(1) Personal data: Your name and surname, residential address, email address, telephone contact, bank account number.

(2) Examples of legitimate interests are: effective defense in the event of a dispute, processing to prevent fraud (eg risk assessment), direct marketing (eg offering relevant services to existing customers), reporting criminal offenses and passing personal data to the competent authority, ensuring network security and information,…

vi) Fulfillment of other legal obligations

(1) Providing information to law enforcement authorities, providing information to public authorities and the like.

b) We process your personal data for the time necessary to ensure all rights and obligations arising from mutual legal action, at least for the period of order processing, trade, service facilities, etc., as well as for the time the administrator is obliged to store personal data according to generally binding legislation or for as long as you may have given your consent. Otherwise, the processing time results from the purpose for which the personal data are processed or is determined by law.

c) Personal data is processed manually and automatically by the administrator. The administrator is entitled to process some information automatically, for example to create statistical information about traffic to the site.

3) Rights of the data subject

a) As a data subject, you have rights determined by law which you can exercise at any time. These are:

i) The right of access to personal data, according to which you have the right to obtain information from the administrator on whether the administrator processes your personal data. The administrator is obliged to provide you with this information without undue delay. The content of the information is determined by the provisions of Article 15 of the GDPR Regulation. The administrator has the right to demand a reasonable payment for the provision of information, not exceeding the costs necessary for the provision of information.

ii) The right to correct or delete personal data, or restrictions on processing, according to which you have the right to have personal data that are inaccurate or incorrect corrected. If your personal data is no longer needed for the purposes for which it was collected or is processed illegally, you have the right to request its deletion. If you do not want to request the deletion of personal data, but only temporarily restrict their processing, you can request a processing restriction.

iii) The right to request an explanation if you suspect that the processing of your personal data by the administrator is contrary to law.

iv) The right to contact the Office for Personal Data Protection in case of doubts about compliance with the obligations related to the processing of personal data.

v) The right to data portability, ie the right to obtain data concerning you that you have provided to the administrator, in a structured, commonly used and machine-readable format, for more details see Article 20 of the GDPR.

vi) The right to object to the processing of personal data which are processed for the purpose of performing a task carried out in the public interest or in the exercise of official authority or for the purpose of protecting the legitimate interests of the administrator. The administrator shall terminate the processing without undue delay unless it proves that there is a legitimate reason/interest in the processing which outweighs your interest, rights or freedoms.

vii) The right to revoke the consent to the processing of personal data at any time if you have given the administrator consent to the processing of personal data.

4) Transfer of personal data to third parties
